Weekend Hospice LPN

Location: Wexford, PA and surrounding areas
Schedule: Saturday & Sunday | 8:00 AM – 8:00 PM
Employment Type: Full Time | Weekend Schedule

PropMed Talent Solutions is partnering with a respected hospice provider seeking an experienced and compassionate Licensed Practical Nurse to join their weekend care team. This role offers the opportunity to deliver high quality, patient centered hospice care within a collaborative and supportive environment.

Key Responsibilities

• Collaborate with the interdisciplinary team to manage and deliver individualized patient care plans

• Provide direct hospice care to patients in Wexford and surrounding communities

• Accurately complete documentation and meet required charting deadlines

• Effectively self manage daily schedule and patient visits

• Utilize tablets, mobile devices, and EMR systems for documentation and communication

Required Qualifications

• Associate degree or higher

• Minimum 2 years of hospice experience

• At least 1 year of experience using an Electronic Medical Record system -including EMR or TruChart EHR

• Active Licensed Practical Nurse license in the state of Pennsylvania

• Valid drivers license

• Comfortable with required local travel

This is a weekend schedule position serving patients throughout the Wexford territory. Local travel is required.
